A Splash Of Humanisation


Hello again. I hope that you are well.  Yet another busy month, as per usual.  My book has now been published and we had a little book launch party type of thing to say thank you to people and celebrate with family, friends and colleagues.   



The book is ultimately aimed to help people and give a little hope.  It was quite therapeutic and emotional to do as I reflected on the last 25 years or so of learning and always looking for better.  Also reflecting on the success stories, many of them life changing and reflecting my team’s core values.  
We have all the fancy stuff with implants, teeth straightening, lasers, 3D scanning, CADCAM, computer controlled, pain free anaesthetic delivery and so much more but probably as important or more so is a BIG SPLASH of humanisation. 
Making A Difference In Morocco
Last week I was back in Northern Morocco  with my charity, Dental Mavericks.  5 days of really hard work but really fulfilling and worth it.  This year I went with Gillian my practice manager (her 5th trip) and Nadene our lead nurse (who was on her first trip and plans to return). We saw over 1200 children in total and took hundreds of abscessed teeth out to get rid of pain and infection.  Every child we saw was taught about the importance of a good diet and how to brush their teeth as well as receiving fluoride treatment to strengthen teeth.  Every child got a toothbrush and toothpaste.  In addition, we did something new for us.  We did SDF therapy, loads of it.

SDF,  What Is SDF?
Well, SDF stands for Silver Diamine Fluoride.  It a liquid that is applied to tooth decay to stop it in it’s tracks. Apparently it has been used in Japan for about 80 years but I only came across it this year and I’m a bit of an anorak.   


It is a quick and easy technique to try and stop and is very good at preventing decay, probably even better than traditional fluoride varnish or gel.  The draw back is that it stains decay and even early decay black.   
We are collecting data and working with Kings College London and Harvard University on the use of SDF.  Now, if we do not remove the decay we cannot place conventional fillings to restore form and function.  What SDF does is allow us to see many people in an attempt to stop disease and prevent further disease, pain and infection. 
A Massive Thank You
A big thank you to everybody who has donated and supported Dental Mavericks over the last 8 years.  It’s been incredible.  Above is some knitting done by our patients and a local Knit and Natter group who are very prolific.  All this stuff goes directly to where there is need. 

Everybody who goes on the missions is self-funded, so it is 100% charity with no idle men with fat salaries.  Proper grass roots humanitarianism.  I’m really proud to be part of it.

The Stem Cell Revolution

Hello again. I hope that you are well. Another busy month. I was at Sheffield University Dental School giving a talk to students at a careers evening.

I tried to inspire them to keep working on themselves and looking for better and stick to their core values. Doing the right things for the right reasons is important and should see you right for the most part.

One thing I did emphasise is “ If you don’t know WHY you are doing something, STOP because you probably shouldn’t be doing it”. The Dental Students Society have chosen Dental Mavericks as one of their charities to support this year which is fantastic.


Heal Thyself

More and more stem cells are being used to treat medical problems and promote healing. Stem cells are cells produced in the bone marrow with the potential to become different kinds of cells because they have not differentiated yet or in other haven’t made their minds up what they want to be when they grow up.



They can be used to repair damaged and lost tissue and promote healing. Dentistry is no different and we are taking advantage of this and using stem cells to help grow bone and gum, particularly in combination with dental implants. We use it in combination with other cells and fibrin.

Exciting Stuff


At Castle Park we’ve invested in a special machine that can centrifuge blood at specific speeds and duration to separate out the stuff we need to speed up healing and reduce inflammation.

This combined with laser therapy means that there is no better way of doing advanced treatment for dental implants and regenerative procedures e.g. developing bone and gum after gum disease or combined with implants. 



Lasers by themselves have been shown to stimulate stem cells within teeth to help the tooth repair itself after removing tooth decay. We use lasers routinely now. I love all this stuff.

Another Mission In Morocco

In just short of 3 weeks time I will be off to Northern Morocco with Gill and Nadene from work with Dental Mavericks to get poor children out of pain. 

We have a collaboration with Kings College Dental Institute, London and Harvard University. More on that next time along with other advantages of the PRF machine.

Knitting With Purpose



Hello again. I hope that you are well. Oh, what a month. My first time giving a speech at an International Medical Laser Congress.

Really exciting, if a bit daunting. It was OK once I got going with some great feedback, which was nice. My colleague and gum expert here at the practice, Ilay, was keynote speaker and started the dental part of congress.

Everybody seemed to know Ilay as he had trained many of them all over the world. There were other parallel sessions going on for other specialities including dermatology, gynaecology, aesthetics and male gento-urinary medicine.

Some of the topics shown the timetable on the big screen in reception for some of these subjects made me wince. I made new, interesting friends from all over the world. I was sat next to Johnny Wong from Hong Kong. What a nice guy. In fact everybody I met was, really positive and enthusiastic.

Yours Truly
I made friends with people from India, Australia, Romania, Canada, USA, Israel, Slovenia, Italy, Hong Kong, England and Singapore. Mostly dental but also with other specialities. I spoke on the Saturday morning and there was a party the night before so there were less people than the day before. I was sensible and got a relatively early night!

Knitting?...Purpose?


Here we go, Branfield off on one again. Just stick with me this is really good. I have mentioned before that two lovely patients knit some fantastic clothes for children that we take to Morocco and send to Syrian refugees in camps and orphanages in Lebanon and Greece. 

Well, a Knit And Natter group in the area got inspired by this and they have been really prolific in knitting clothes and blankets for these needy people. I think that this is so great, people getting inspired to get involved doing good for people they don’t know who have nothing for nothing. That is what our Dental Mavericks Charity is all about.

Having A Catch Up Over Tea And Cake 



So, we finally met one sunny evening just recently as I wanted to say a personal thank you and show them what Dental Mavericks was about with pictures and tell the story of how it happened and illustrate with a few case study stories.

My colleague made the cakes that went down a storm (thanks Gail for all your hard work and organising). 



So, once again, a MASSIVE THANK YOU to the Knit And Natter group for your support and our anonymous patients for all that you do. I’ll be off to Morocco in October for a big project in the Rif Mountains. 



Becoming A Bigger Player


In the Lebanon Dental Mavericks have now a reputation for being very much people and caring based and making things as fun as possible in such awful circumstances. Now working with other NGO’s and even involved with research with King’s College, London and Harvard University, Boston, USA. Well done Cally Gedge for driving this and your energy and love. Well proud!

Hello again. I hope that you are well. Well, as usual a busy time. Dental Mavericks were approached by Sheffield University Dental School. The Students have to do an elective whereby they spend time somewhere, home or abroad and write up their aims and objectives and what they found. This can be scientific or socially based but to do with dentistry. I was daft enough to say that I would give a presentation to them.



Don’t Give Up The Day Job

So, I took a day off work to go to Sheffield. There were other speakers, mainly corporate companies that specialise in these elective “holidays” for students. I was on last. Once I got going I quite enjoyed it. The students seemed to engage and I got great feedback. I was even approached to see if it was OK for the Sheffield University Dental Students Society to support Dental Mavericks. Wow, that was a result!!! If I could just inspire some youngsters to get involved and come with us that would be great. They are the future.




Back At The BBC

I was also asked to be on BBC Radio Humberside again. The sugar tax was introduced last month. Andy Comfort interviewed me over the phone. He was a really nice guy. There is far too much sugar in all sorts of stuff and there is an increase in child dental decay and teeth out as well as a worldwide general health problems associated with empty calories due to excessive sugar intake.

So, Back To Snoring, As Promised

Snoring affects 40% of us, and can have mild to devastating effects on our health, from general tiredness to increased risk of hypertension and subsequent cardiovascular problems.

Lasers Can Help

One way to improve the airway is with a GENTLE laser treatment, a process called Nightlase™. Sometimes surgery is offered but it is really painful, takes a long time to heal creates scarring that can effect swallowing and sometimes speech.

With the Dual wavelength Nightlase™ treatment the low laser energy creates a gentle heating of the tissue of the soft palate. There is no need for local anaesthetic or a recovery period, so no downtime afterwards. Just get on with your life. You need a full assessment first as usual. One assessment is called a Mallampati score. The idea is to get you to a low Mallampati score to open up the airway. It is usually 3-5 treatments about 30 minutes each 21 days apart.



The gentle heating is about 50-65 degrees. This equates to a cup of tea hot, so can feel a bit scratchy. The heat causes tissue changes with shrinkage from day one usually. There is a change in he collagen that helps with this. The effects can last a year or so with no tissue damage or scarring. Mallampati

An Interesting Fact

This science was first used in gynaecological and female genito-urinary medicine to reduce urinary incontinence and other stuff. It was some smart Argentinians that thought of applying the same principles to anti-snoring treatment. I have no pictures of any of this.

Guest Speakers

Hello again. I hope that you are well. Happy New Year to you and sincere best wishes for 2018.  Hope had a great Christmas.

I thought I’d start the year on a real positive note.  Here goes.

Stop The Waffle

Now I am a self-confessed dentistry geek/anorak.  I do waffle on a bit and get excited about modern advancements and the science of dental stuff. So……….I thought I’d give you a rest.

Going Global

Some of you might know that I am a Trustee and founder member of a Charity called Dental Mavericks.  We get people, mainly children, who have no access to dental services, out of daily pain.  We have done this in Morocco since Sept/Oct 2010. In December 2016 we went to Thessaloniki in Greece to treat refugees.  This has been an on going project since.  Dental Mavericks, in particular Cally Gedge, who organises pretty much everything, are now helping organise emergency dental treatment for Syrian refugees in the Lebanon.  Now, I went to Greece but I don’t think my better half will give me a pass for the Lebanon.  Fair do’s.





Knitting With Real Purpose

I have some lovely patients who are great at knitting and have knitted loads of stuff for children in Morocco.  In Lebanon these baby clothes have gone down a storm.  Received with much gratitude.  The refugees live in camps in absolute squalor and have nothing.  This is such a fantastic thing to do for someone that you do not know and will never meet to help make their lives better.









Life is like the weather:  there is rain and grey cloud with intervals of sunshine.  This is pure sunshine.  When the sun shines we should endeavour to bask in it and not close the curtains and stay in.  So let’s enjoy this moment of sunshine and be grateful for what we have.  Also a big thank you my knitting Sunshine Makers!!  There is so much anger, pain, discourse and division going on.  This is such a great antidote.

On Top Of The World


Hello again. I hope that you are well. Another busy month, phew! I’ve just got back from Morocco getting kids out of pain up in the Morocco . This was a collaboration with the Local Health Authority in Chef Chaouen province.

It Felt Like Being On Top Of The World

We got more remote than ever before needing 4x4’s to get us to some places. We all worked really hard and put our heart and souls into it. Another great team of people and our largest team yet. In fact our British team split into 2 teams for 4 out of the 5 days of working so we could get to as many people in need as we could. We saw almost 3000 children and every one received fluoride treatment, oral health advice, a toothbrush and toothpaste. In fact with our Moroccan colleagues about 6000 kids received this. We actively treated many hundreds children in pain and took thousands of abscessed teeth out. Bless ‘em.




Not 2 Halfpennies To Rub Together

People here are really poor and there has been a lot of resentment towards the government. Everybody we worked with was so friendly and appreciative though. The kids, teachers and parents were lovely.



And Breathe

Communication and rapid trust building with the kids is always key. We did have lots of help from interpreters when required but the main thing is keeping eye, verbal and even tactile contact and always asking if they are OK (“cava or labass”). The other thing that is massive to show empathy and understanding is breathing. This works wonders for relaxation. We all forget to just breathe and just “be” to relax. I do not know how to spell this but the word for “breathe,” phonetically is “sniffez”. We were all saying and doing it and really elaborating on it to add a bit of humour. “Humanisation”.



Not Lost In Translation

We discussed the role of interpreters and it works best just to do the minimum to allow an “in “ for the dental team so they can start and develop a trusting relationship, heart to heart, human-to-human. One young Moroccan interpreter got it when I explained. He said “emotional content”. Bob on!! It’s all about that and humanising every step of the process, stripped right back to basics. Formal language is then secondary.

A Massive Big Thank You

A heart felt thank you to the ladies who give me knitting to take out to the poor families. You know who you are! Also thank you to everyone who has donated money, toothbrushes and toothpaste. It’s great for us all to be involved in helping people and really trying to leave a legacy of impact on peoples lives for the good.

Certificate Time

As I mentioned this was a joint venture with the local authority and many bridges were built. We had to sit through a really long speech and ceremony of certificates. My name was spelt wrong. I should have done my usual ,“ think branflakes but field instead”.

A Bit Of Hard Work Does You Good


Hello again. I hope that you are well. Just back from Morocco with Dental Mavericks getting people out of daily pain. My 12th trip, I think. You would think that you just get used to it but it is always emotional.  

My wife picked me up from Hull train station and to my amazement BBC radio Humberside was on in the car and it was the football commentary (my wife doesn’t do football). She was gripped by the drama of it at the end of the season. However, it was not to be for Hull City to stay up in the premier league. Here we go again! There is now the possibility of more drama and excitement trying to get back and even possible Wembley trips again. Enjoy the journey of life!

We Were Absolutely Whacked





This trip to Morocco was backed by the King’s advisor so had a licence to treat adults as well as children. There were so many people! My word, I have never worked so hard in my life. My colleagues said the same. When you treat children there it is tiring but more on an emotional level. This is because they are children having teeth removed and doing our best to look after them with a language barrier demands total concentration in this regards to building a relationship with constant eye contact, verbal contact, body language and reassurance. It is like this with adults there and with our patients at home in Cottingham but more intense over there with the children I feel. It’s all about PEOPLE. Now with throwing adults into the mix there are the same challenges but also some very difficult tooth extractions, more often than not multiple. Well, with one after the other all day your body starts to feel it. I’m not getting any younger you know. I’m not joking; some of us were ready to drop at the end of each day.



Getting Rewards

The most rewarding for me is when we went rural up into the hills. There is more proper poverty here and people cannot access a dentist at all. We saw a lot of Berber children here. Our translators had to learn a bit of Berber as they did not speak Arabic, and then teach us a few words. These were so lovely, very stoic and appreciative. One little boy I treated had 4 teeth out in one go. He was only 6 yrs old and afterwards he hugged me and clung on like a limpet, he had a smile from ear to ear. It really touched me. This is when you know you’re doing the right thing when it means so much to be out of pain.



Unusual Name

One of the Berber kids had an unusual name. He was called Tagine. Our Moroccan translators had never heard this name before and thought it rather special. Pretty much every meal we had was a tagine of something. His parents must like their food eh?

Time For A Bit Of Awe

We have couple of lovely ladies that have done kitting for us to take out with us o give to the needy. Now, I didn’t go to the maternity but a couple of midwives in our group did. It was a far cry from the facilities we have here. Babies everywhere and not very clean. They distributed some of the lovely garments that Ann had made so there will be some cosy warm babies. I must say that I would wear some of the stuff if it was in my size.

A Bit Of A Hoo-Ha

            

Hello again, I hope you are well.  I can’t believe it’s three weeks into January already.  We’ve hit 2017 at quite a pace and there are loads on. Tomorrow my team and I are off to Manchester to a Dental Customer Care Conference and a Charity Ball to raise money for Dental Mavericks.  We are also busy organising our second 70’s disco fundraiser for Dental Mavericks at Armstrong’s Social Club in Beverley on February 10th.  There might be a few tickets left if you fancy it.  Last year was a good laugh.

So What’s All The Hoo-Ha

Well, it’s toothpaste.  Colgate toothpaste to be more precise. There is an anti-bacterial chemical called Triclosan that has been in Colgate Total Toothpaste since about 1997.  This toothpaste was considered the benchmark in modern toothpastes because if it.  Triclosan was also put into hand soaps and gels as an anti-bacterial agent.  Now here’s the rub.  Quite recently the American Food and Drug Administration (FDA) banned Triclosan from soaps and gels.  The reason is on health grounds.  It was found that Triclsan is absorbed through the skin and can cause endocrine problems and also affect he immune system.  Other studies have shown it to cause tumours in mice.  Now mice are not humans so there isn’t a direct correlation.  However, Triclosan is still in Colgate Total toothpaste for some reason even though the mouth allows quicker absorption.  Colgate argued that there are loads of studies to show its safety.  The FDA took this board and concluded that the benefits may well outweigh the risks.  Now, Triclosan is not in Colgate sensitive as far as I know.  I’m not trying to scare monger at all, I just find it all very interesting and thought you may want to know or research yourself.

Now For Some Optimism

Fillings could be consigned to history after scientists found that a drug given to Alzheimer’s patients can help teeth regrow and repair cavities. Researchers at King’s College London found that the drug Tideglusib stimulates the stem cells contained in teeth so that they generate new dentine, the material under the enamel.  They discovered that the drug heightened the activity of stem cells in the dental pulp so they could repair 0.13mm holes in the teeth of mice.
A drug-soaked sponge was placed in the hole and then a protective coating was applied over the top. As the sponge broke down it was replaced by dentine, healing the tooth.
Now, don’t get over excited just yet, a 0.13mm hole is very big and easy to protect.  I am intrigued to see how it fairs with bigger holes and how they protectively coat these so there is no wash out of the drug.  It all looks very promising though.

There Is Still Hope

I saw Hull City beat Bournmouth 3-1 last weekend.  It was great!.   There is still hope of staying up if we can keep sticking it in the net.   Come on lads, get stuck in.  Oh, I thought the City Of Hull Celebrations on Queen Victoria Square was really good. Very stirring.

Wow, what a month. I managed the “Coast To Coast” bike ride from Barrow- in Furness to Whitby. By ‘eck it was hard work. Hill after hill after hill. All 3 days into the wind, often blustery and strong, some torrential rain and white out with mist at the top of Tan Hill. A well-earned half -pint at the top with some hot soup and chips. I’m really pleased to have raised about £900 for my Dental Mavericks Charity.

Five days after the bike ride I flew out to Marrakech before my transport to Asni in the foothills of the Atlas Mountains. Here I met up with colleagues, most of which I had never worked with before and some I had never met until then.
 
This was a brand new project working with the Eve Branson Foundation getting children out of pain.

I have been to Northern Morocco four times before and was hoping that the teeth of the children in this different part of the country would be better.

This was not the case. There were so many decayed teeth and we had to use our limited resources to concentrate on getting children out of pain by removing bad teeth.

New Records Set

Just wanted to get on with her colouring in.
Amazing
On day one we were told that there were 30 children to see. We actually saw 130 kids. Word was out and they were walking 2-3 hours for teeth out to end daily pain. We were taking teeth out for children from as young as 3 years old up to 16 years. We soon worked together as a natural team and I am proud to have bee part of it. This was “pain free” dentistry at its best. In 2 days we saw 270 kids and we took 140 children out of dental pain and removed 325 bad teeth. Phew!

Emotion Nearly Got The Better Of Me


 
There was a young lad called Mohammed. He was early teens and had 2 adult molar teeth that were rotten down to bone and not a lot left. He was in pain and wanted them out. My colleagues tried but, unfortunately, couldn’t get near him to even put in the little cotton wool rolls with sleepy gel.

I was asked to “have a go”. I did connect with him and did manage to make a start getting him numb. It did

We did a lot of deep breathing together and I said “Cava?” a lot. We got 3 teeth out on one side. They were really tough but we went really gently and did not rush. Mohammed needed a rest, so did I. We hugged each other for a bit before getting the last tooth out. He was so appreciative. I nearly caved in and nearly blubbed when an interpreter said that Mohammed said I was a good man. We were all humbled by the appreciation we received.

A Great Platform To Work From

I am really pleased with the collaboration with the Eve Branson Foundation. We were looked after really well and they were really chuffed with us as we exceeded all expectations. This is a great platform to progress and we can support each other to provide a “Teeth For Life” programme for the community.

transpire that Mohammed had had a head injury that did leave a bit of brain injury. We were doing really well before he got a bit agitated again. I thought that we would need to give in as I feared for our safety. Bless him though, he was adamant that he needed his teeth out and he filled up. I gestured to him ‘one more go’ and off we went again.
